to manage complex issues experienced by those with a disability. We meet with these organisations to identify issues, develop and implement strategies that ensure positive outcomes are achieved.
We work with participants who are transitioning from a healthcare facility to home ensuring all areas of need are clearly identified and addressed prior to and post discharge. This ensures a smooth transition and continuation of expert care into the future.
We liaise with outpatient services ensuring a link between health organisations and community providers is maintained so participants are aware and participate in their healthcare outcomes.
Clinical Support

Our clinical team

will work alongside you and your support network to design, review and adjust personalised care plans related to your day to day care. This includes but not limited to continence, wound care, diabetes, palliative care, and other complex health and disabilities.

Using our extensive comminity provider network we can assist you to find services to fill existing gaps, e.g. Support Coordination, Occupational Therapy, Physiotherapy, Speech Therapy etc. thereby ensuring you are provided optimal health and wellbeing outcomes.

Practice hours
Monday to Friday 8:00am – 5:00pm
After-hours phone support can be arranged.
Telehealth appointments are available for assessments and some follow ups depending on access for face to face services, funding and regional and remote locations
